We will assist in verifying benefits and submitting claims to your insurance provider; however, coverage and reimbursement decisions are made solely by your insurance company. It is important to understand that not all services may be covered under your policy, and any portion not paid by insurance, including co-pays, deductibles, or non-covered services, is the responsibility of the patient. If your insurance provider denies coverage, you will be responsible for the full balance of the services rendered.
Please ensure that you understand your policy's terms and consult your insurance company regarding any questions about coverage or payment. Payment is due at the time of service, unless other arrangements have been made in advance.
